Research Ethics Application – Pilot V0.0 : This is the programmed / intelligent Word document that should be used for completing applications to Research Ethics Committees. N.B. It is essential that you read the Instructions document (referenced below), particularly the Technical Considerations at the start of the document, before starting to use the Research Ethics Application document.

Instructions For Fifth Draft And Pilot : This pdf document outlines the technical considerations & guidelines for completing the application form, and detailed instructions for each section and question. This document can be used with either the Programmed application form (Pilot V0.0) or the non-programmed, standard Word application form (Draft Five).

Fifth Draft Questions Only : This form should be used if the applicant has technical difficulties in using the programmed Pilot V0.0 form. The Instructions document has guidelines and help for each question, but this form does not automatically remove irrelevant questions for the user.

The Pilot V0.0 and Fifth Draft documents are mutually exclusive and only users who have technical issues preventing them from using the Pilot form will need to use Draft Five.
